Which box do I check for “Submission Status”?
A new submission is one that you are submitting for the first time.
A resubmission is one that has previously been submitted and now is being submitted again. For example, modifying a product from a prototype to production or modifying a product that was ruled nonconforming. If your product is a resubmission, be sure to enter the previous decision # in the “Resubmission Decision #” field.
A replacement is one that needs to be submitted again due to circumstances such as damage to the product.
Which box do I check for “Type of Submission”?
A mockup is a non-functional model that is made to resemble the shape of the product.
A prototype is a first, typical or preliminary model of something, from which other forms are developed or copied.
We consider a first article to be a sample manufactured prior to the start of production for the purpose of ensuring that the manufacturer is capable of producing a product that will meet specified requirements.
A production model would be one that you would offer for sale to the public or to be used for play. The main difference from the other 3 categories above, is that the final production pieces that would be played in competitions and marketed, would get an official conformance ruling.
What do I enter for product composition?
This is the material that makes up your product (i.e., steel, aluminum, rubber, etc.)
What is “Publish to Web Date”? (Club Submission Form)
For all clubs other than putters, this is the date that the club will be shown on the List of Conforming Driver Heads or the Informational Club Database on the USGA website. Please be sure to enter the date on the form. If it is left blank, it may not be published.
Where is the credit card number field?
For security purposes we ask that you do not enter a credit card number on the form. We ask for the name and address of the credit card holder so that we can set up an invoice. Upon receipt of your submission, our accounting department will email a secure link to make payment online.
If I submit a prototype or first article, do I need to later submit a production sample?
For prototypes or first articles, we give an opinion advising if your design does or does not appear contrary to the Equipment Rules. To obtain an official conformance decision, you must submit a production sample.
